Hypertension RPM — Support

Help with the Hypertension RPM app, operated by Mount Sinai Heart as part of a physician-supervised remote patient monitoring program for adults with high blood pressure.

Medical emergency? Do not use this app or this page. Call 911 or go to the nearest emergency department. The app is not monitored in real time and is not a substitute for emergency care.

Common Questions

How do I get an account?

Enrollment is initiated by a healthcare provider. The app is not intended for use without an invitation from the program. If you believe you should have access and cannot sign in, email support@htnprevention.org from the email address you gave the program.

I did not receive my sign-in code

The app signs you in with a one-time passcode sent by email or text message — there is no password. If the code does not arrive:

If it still does not arrive, email us and include the email address or phone number you are trying to use.

My blood pressure cuff will not connect

The app connects to your cuff over Bluetooth. If pairing fails:

You can always enter a reading manually while you sort out a connection problem — a manually entered reading is still visible to your care team.

A reading is missing from my history

Readings taken while your phone is offline are saved on the device and upload when the connection returns. Open the app with a working network connection and give it a moment to sync. If a reading is still missing after that, email us with the approximate date and time.

I am not getting reminders or notifications

Check that notifications are enabled for the app in your phone's settings, and that Do Not Disturb or a Focus mode is not suppressing them.

How do I update my name, contact information, or insurance details?

You can update your profile in the app. Some changes are reviewed by program staff before they take effect. If you cannot make a change in the app, email us or contact your care team.

How do I delete my account?

Email support@htnprevention.org with the subject line "Account deletion request" from the email address associated with your account, or contact your care team. We will verify your identity and process the request. Some data must be retained as required by HIPAA, payer rules, or other law — see the privacy policy for details.

Does the app cost anything?

There is no charge to download or use the app, and there are no in-app purchases. Remote patient monitoring is a clinical service that may be billed to your insurance; your care team can answer questions about coverage and billing.

How is my information protected?

The app handles Protected Health Information and is operated in compliance with HIPAA. Our privacy policy describes what we collect, how it is used, and who it is shared with.

A note on our other app. We separately publish the HTN Prevention Program app for a first responder population. That is a distinct program with its own enrollment path and support contact. This page covers the Hypertension RPM app only.
